Case Management (Community-Based Rehabilitation)
This service helps individuals build skills and connect with resources that promote independence, community inclusion, and quality of life.
Through this service, individuals:
- Experience reduced mental health symptoms
- Build adaptive skills that increase independence
- Access and coordinate supports, including vocational and medical services
Therapy
Our therapy services are provided by counselors and social workers with specialized training in supporting individuals with intellectual and developmental disabilities (IDD). Therapy approaches are adapted to meet the learning styles and communication needs of individuals with IDD.
Therapy is engaging and interactive, using tools such as games, role-playing, rehearsal, and visual supports to help individuals understand and practice new skills. We also work closely with caregivers to reinforce progress and support consistency across settings.
Our therapy services support individuals experiencing anxiety, depression, grief, trauma, and other emotional challenges.

- Build greater independence in daily life
What is Trauma Therapy?
A Story of Healing: Meet Charlie
Charlie is 25 years old and lives with his aunt. Earlier in his life, Charlie experienced some very scary events that were difficult for him to talk about.
With the support of his therapist, Charlie is working on healing from these experiences. Because talking about feelings can sometimes feel overwhelming, Charlie and his therapist are creating a trauma scrapbook together. This helps Charlie express his thoughts and experiences in a way that feels safe and comfortable for him.
Through activities like this, Charlie is beginning to better understand his feelings, recognize his strengths, and see the progress he is making in his healing journey.
Trauma JAR Module (Journey About Resiliency)
One way our therapists support healing from trauma is through the Trauma JAR Module (Journey About Resiliency).
In this trauma-informed approach, individuals participate in guided activities that help them explore and process their experiences as they engage in therapy. After each activity, participants reflect on what they learned about themselves and place their written reflection into a jar.
Over time, the jar becomes filled with reminders of personal strengths, insights, and progress made throughout the therapy process. These reflections help individuals recognize their growth and build confidence as they continue their healing journey.
Psychiatry
For individuals with intellectual and developmental disabilities (IDD), symptoms such as anxiety, depression, or pain may sometimes be inappropriately attributed to a person’s IDD rather than recognized as a separate mental health concern. This is known as diagnostic overshadowing.
Our psychiatry services are provided by Psychiatric Mental Health Nurse Practitioners (PMHNPs) with
specialized expertise in identifying and treating mental health symptoms in individuals with IDD, helping ensure that concerns are properly evaluated and addressed.
Our psychiatry services include:
- Comprehensive evaluations to determine if medication may help manage symptoms
- Ongoing monitoring of symptoms and medication effectiveness
- Education for individuals and families about medications and potential side effects
Appointments are available in-person or through telehealth.
Behavioral Health Nursing
This program is available by referral from your Psychiatric Mental Health Nurse Practitioner and is designed for individuals who need extra support with medication consistency and health management.
The program is delivered by a Registered Nurse (RN) or Licensed Practical Nurse (LPN) and includes:
- Physical and preventive health screenings
- Medication management, monitoring, and education
- Medical care coordination and illness management
This program is provided in the home or community.
