Waiver Services Through the Ohio Department of Developmental Disabilities

Empowering Integrated Care Solutions is certified by the Ohio Department of Developmental Disabilities to provide Home & Community-Based Waiver Services for individuals with intellectual and developmental disabilities.

Our waiver services are designed to support independence, daily living skills, community inclusion, and overall quality of life through individualized, person-centered care.

We collaborate closely with County Boards of Developmental Disabilities, caregivers, and interdisciplinary providers to ensure coordinated and individualized support.

Individuals may qualify for waiver services if they:

  • Are age 18 or older
  • Have an active Individual Options (IO), Level One , or SELF Waiver
  • Have an established intellectual or developmental disability
  • Have active Medicaid coverage or are Medicaid eligible


Waiver services are authorized through the local County Board of Developmental Disabilities and provided at no cost through Medicaid Waiver funding.

Homemaker/Personal Care (HPC)

Homemaker/Personal Care (HPC) supports individuals with activities of daily living and independent living skill development. Support may include personal hygiene, meal preparation, cleaning, laundry, shopping, money management, and community participation.

 

Supports are individualized based on each person’s needs and coordinated with the broader care team to promote independence and daily living success.

HPC Transportation Services

HPC Transportation supports access to appointments, community activities, resources, and other service-related needs that promote independence and community inclusion.

The amount and frequency of transportation are determined through the County Board Individualized Service Plan (ISP) process based on the individual’s assessed needs.

Social Work Services (IO Waiver Only)

Licensed social work professionals provide support with community resources, service coordination, advocacy, caregiver support, and adaptive skill development to help individuals improve daily functioning and overall quality of life.

Social Work services are also provided per authorization of the County Board of Developmental Disabilities.
